The first signs of Perlman syndrome may be detected in pregnancy as when a prenatal scan can reveal excessive amniotic fluid (polyhydramnios) and a large fetus. In newborn babies, a very high birth weight is frequent, and the kidneys and liver enlarged. Doctors may detect similarities in facial appearance in children with Perlman syndrome that can cause them to suspect the diagnosis.
